Form Builder

The Form Builder is where you design an intake form from scratch or edit one you already built. Set the title, description, default language, then add the fields one at a time and pick their type.

   Form title

   Required. The name of the form.

   Description

   Optional. Short blurb that shows above the form fields.

   Default language

   Two-letter language code (en, es, fr, etc.) — used when translating field labels.

   Field label

   The text shown above the input on the live form. Required.

   Type (Text / Select / Date / Checkbox)

   Picker for the field's input type. Opens a menu with the four choices.

   Options (comma-separated)

   Shown only for Select fields. Type the choices separated by commas.

   Required switch

   Toggle whether the field must be filled before the form can be submitted.

   Remove field (trash)

   Deletes that field row from the form.

   Add Field

   Adds a blank field row at the bottom.

    Save

    Saves the form. Validates that every field has a label before submitting.
