Intake Form Builder

The Intake Form Builder lets you design a form with any number of fields. Pick a source language, add labels, choose field types, and save. Vavus translates each response back to your source language automatically.

   Cancel

   Discards your changes and returns to the Intake Forms list.

   Save form

   Saves the form. New forms appear on the Intake Forms page and get a shareable link.

   Title

   The name displayed at the top of the form.

   Description

   Optional sub-line shown under the title.

   Source language

   The language you'll see responses in (and the language the form is authored in).

   Add field

   Adds a new question to the form (text, number, choice, etc.).

   Move up / Move down (per field)

   Reorders the field in the form.

   Delete (per field)

   Removes the field.

   Field label / type / options

   Edit the question text, choose the input type, and (for multi-choice) list the options.

    Add another field

    Same as Add field — appears at the bottom for convenience.
